Unison Health and Community Services is offering free ongoing primary care services.
We are currently accepting individuals:
- who do not have a primary care doctor or nurse practitioner (top priority), or
- who are looking for a new primary care doctor or nurse practitioner.
We offer these services within a geographic area in Northwest Toronto.
Individuals who do not have OHIP or IFH could be eligible. We will assess your eligibility during intake.
